After days of investigation on the latest incident on a work issue, Johnny finally decided to come clean with the task force which was setup to understand what had gone wrong from the start that led to the severe monetary losses borne by the company.
This incident is due to a badly made decision which resulted in many downstream processes not being able handle this change as it was poorly informed. When being asked by the task force why was this change not communicated downstream early, Johnny finally shared that he was coerced by his supervisor not to share early as the team had other priorities on hand to handle and this decision was only made when the change had to be done instead of planning ahead and ensuring all downstream is ready to take the change. He mentioned he did tell his supervisor early, but his supervisor was engrossed with other task. And when they knew they had to administer the change, even when Johnny asked to hold the change until downstream is properly communicated, in view of time, his supervisor coerced him to just go ahead.
The next day, Johnny's supervisor is summoned to the task force for questioning.
