Thursday, the last workday, wasn't busy except for one hour in the afternoon. My evening shift started at 5 p.m., and I already had two patients waiting for me. When I passed by one patient, I heard a wheeze coming from him. I called him without any further delay. It was a case of acute exacerbation of bronchial asthma. Surprisingly, his vitals including oxygen saturation, BP, and pulse were all within normal limits. He was producing wheeze and chest auscultation revealed marked abnormalities. I managed him with nebulization and injections, followed by oral medicines and an inhaler.
The next patient was worse than the first one. He was severely breathless with low oxygen saturation and quite a high heart rate. Besides nebulization and injections, I added oxygen as well. When he left, his breathlessness improved but oxygen saturation didn't.
Another patient came with acute right upper abdominal pain with vomiting. It was a suspected case of GB problem (either infection or stone). He required an ultrasound scanning. As we didn't have one, I referred him to the higher center.
A few more patients visited at that time as well. After breakfast, there were only one or two patients. Altogether, I consulted 18 patients on Thursday.
